Boone County has a large number of dedicated healthcare professionals, many of whom never receive a formal award. Witham Health Services is sponsoring the annual Healthcare Professional of the Year award. The award will be presented Nov. 11 during the Boone County Chamber of Commerce Annual Dinner and Civic Banquet.
The Healthcare Professional of the Year Award is designed to highlight the best in Boone County. The individual can work in any aspect of the healthcare industry; including, but not limited to, registered nurse, certified nursing assistant, physician, emergency medical technician, paramedic, imaging technician, pharmacist, social worker, program director, dietary and housekeeping staff. The Healthcare Professional of the Year must demonstrate proficiency in patient care as well as an attitude of professionalism which commands respect from his or her affiliation, coworkers, patients and the community he or she serves.
The purpose of this award is to provide recognition for those contributions which are above and beyond the normal day- to-day work activities and which make a lasting impression or impact on the lives of patients and their families. Contributions to healthcare in Boone County through dedication, service and leadership to the community through promotion of healthy living, public health education and volunteer activities are all considered in the selection of the award recipient.

Your nomination should:
1. Provide examples of how the candidate demonstrates his/her knowledge and skills relating to the delivery of patient care.
2. Describe activities of the candidate that demonstrate a degree of professionalism, eagerness
to serve and quality of leadership which goes beyond the expected norm in his or her
organization.
3. Provide examples of the candidate’s contributions to healthcare within his or her community which promote healthy living, public health education or volunteer activities.
4. Explain in your own words why you think the candidate should receive this award.
All nominations must be received by 4 p.m. Oct. 8.
